If you've been diagnosed with Hashimoto's, there's a good chance your treatment has centered around one thing: your thyroid. A prescription. A follow-up blood draw. A TSH number that's supposed to tell you whether everything is under control.
And for many people, thyroid hormone replacement is an essential and effective part of treatment.
But what happens when the lab results look good and you still don't feel good?
I spent years asking myself that question.
For a long time, Hashimoto's was the only explanation I had for what was happening in my body. I assumed the fatigue, anxiety, brain fog, changes in sleep, pain, and periods where I simply felt unwell all belonged to the same thyroid problem.
As I learned more about my health — and eventually received additional diagnoses — I realized the picture was much more complicated.
That doesn't mean Hashimoto's explains every symptom. It means Hashimoto's and hypothyroidism are not the same thing, and one lab number can't always explain an entire human body.
What Hashimoto's Actually Is
Hashimoto's thyroiditis is an autoimmune disease. That means the underlying problem begins with the immune system, which mistakenly targets thyroid tissue. Over time, this immune-mediated damage can interfere with the thyroid's ability to produce enough thyroid hormone.
That can eventually lead to hypothyroidism.
This distinction is important:
- Hashimoto's is the autoimmune disease.
- Hypothyroidism is the hormone deficiency that can result from it.
You can have hypothyroidism without having Hashimoto's, and Hashimoto's can be present before thyroid hormone production becomes significantly impaired.
That was something I didn't fully understand when I was first diagnosed. To me, Hashimoto's simply meant, "I have a thyroid problem." Now I understand that the thyroid is the organ being targeted — but autoimmunity is the process behind it.
When the Numbers Look Better but You Still Don't Feel Better
Thyroid hormone replacement can correct the hormone deficiency caused by an underactive thyroid. For many people, that significantly improves symptoms.
But some people with Hashimoto's continue to report fatigue, cognitive difficulties, mood changes, pain, or a reduced quality of life even when thyroid hormone levels have returned to the expected range.
Researchers are still trying to understand exactly why. And this is where I think we need to be careful.
Persistent symptoms do not automatically mean:
- Your thyroid medication isn't working
- Your immune system is secretly causing every symptom
- Your inflammation is out of control
- Or your doctor is missing Hashimoto's activity
There can be many other contributors. Sleep problems, anemia, nutrient deficiencies, hormonal changes, medications, other autoimmune diseases, chronic pain conditions, mental health, dysautonomia, and completely unrelated medical conditions can all produce overlapping symptoms.
That's why I no longer think of a normal TSH as either "everything is fine" or "the labs must be wrong." Sometimes the more useful question is: what else belongs in the picture?
My Hashimoto's Never Felt Like Just a Thyroid Problem
This is where I want to separate medical evidence from my own experience. In my body, Hashimoto's never felt limited to my thyroid.
I experienced fatigue. Anxiety. Brain fog. Sleep disturbances. Pain. Periods where my body simply felt inflamed, reactive, and difficult to predict.
For years, I tried to understand all of that through the lens of Hashimoto's because it was the diagnosis I had. I would have a flare in symptoms and think, my thyroid must be acting up. My anxiety would increase and I would think, I must just be stressed. My heart would race and I would treat that as another unrelated issue.
I had separate explanations for everything. What I didn't have yet was a way of looking at my body as an interconnected system.
Then More Pieces of the Puzzle Appeared
I was diagnosed with Hashimoto's in my 20s. It wasn't until my 40s that MCAS, POTS, and EDS became part of my medical picture.
Suddenly, symptoms I had spent years placing into separate boxes didn't feel quite so random anymore.
That experience changed the questions I started asking. Not, "Did Hashimoto's cause all of this?" — because I don't believe the evidence supports saying that. Instead: "How might these different conditions, systems, and symptoms be interacting in the same body?"
That is a very different question. And honestly, it changed everything about how I understand my health.
The Immune System Doesn't Work Alone
One of the biggest lessons I've learned is that body systems are constantly communicating. The immune system interacts with the nervous system. The nervous system influences digestion, cardiovascular regulation, sleep, pain processing, and stress responses. Hormones influence multiple tissues throughout the body. The gut communicates with both the nervous and immune systems.
None of that means there is one hidden "root cause" behind every chronic illness. And it does not mean nervous system regulation, diet, supplements, or lifestyle can cure autoimmune disease.
It means the body is interconnected.
That concept seems obvious once you understand it, but healthcare is often organized by specialty. A thyroid problem goes to endocrinology. Palpitations go to cardiology. Digestive symptoms go to gastroenterology. Pain goes somewhere else.
Each specialist may appropriately focus on their part. But when you are the person living inside the body containing all of those parts, they don't feel separate.
What About Inflammation?
Hashimoto's involves an immune-mediated inflammatory process in the thyroid. But I would be cautious about assuming that every persistent symptom in Hashimoto's is caused by "systemic inflammation." That is something I used to believe much more strongly.
Now I think the more accurate position is: inflammation may be part of the picture, but it isn't automatically the explanation for everything.
That distinction matters. Because if we assume every symptom is inflammation, we may stop looking for another explanation that deserves attention. And that's the opposite of what Regulating The Chaos is about.
I want to connect dots. But I don't want to connect dots that science hasn't connected yet.
Hashimoto's and Other Autoimmune Diseases
Another important part of the picture is that autoimmune diseases can cluster. Having one autoimmune disease can increase the likelihood of developing another autoimmune condition compared with someone without autoimmunity.
That does not mean everyone with Hashimoto's will develop another autoimmune disease. And it does not mean every chronic condition that occurs alongside Hashimoto's is autoimmune.
That distinction matters particularly when discussing conditions like POTS, EDS, and MCAS. They may coexist in some people — including me — but they are not all autoimmune diseases, and science has not established one single shared mechanism that explains why all of them appear together in certain individuals.
That is an area where there is still much more to learn. And I think saying "we don't fully know yet" is much more valuable than pretending medicine already has an answer.
What Changed for Me
For years I approached every diagnosis and symptom as its own problem. Thyroid. Anxiety. Heart rate. Pain. Digestive symptoms. Immune reactions. One body, but completely separate explanations.
Eventually I stopped asking, "Which diagnosis does this symptom belong to?" and started asking, "What systems are involved here?"
That shift has helped me understand my body with much more curiosity. Sometimes the answer is still my thyroid. Sometimes it isn't. Sometimes there are several things happening at once. And sometimes the answer is simply: we don't know yet.
I'm much more comfortable with that than I used to be.
What We Know — and What We're Still Learning
We know Hashimoto's is an autoimmune thyroid disease. We know it can damage the thyroid and eventually cause hypothyroidism. We know thyroid hormone replacement is an important treatment when thyroid hormone production becomes inadequate.
We also know that some people continue to experience symptoms even after their thyroid laboratory values normalize.
What we do not fully understand is why every person experiences Hashimoto's differently, why certain symptoms persist in some people, or exactly how Hashimoto's interacts with every other condition that may exist in the same body.
And that uncertainty is important. Because people living with chronic illness deserve better than two extremes: "your labs are normal, so nothing is wrong," and "every symptom you have is your Hashimoto's."
There is a lot of room between those statements. That is usually where the real conversation needs to happen.
The Bottom Line
Hashimoto's is more than hypothyroidism. It begins with autoimmunity and can eventually lead to thyroid hormone deficiency. But having Hashimoto's does not mean every symptom you experience is caused by your thyroid — or even by autoimmunity.
Your thyroid labs matter. Your symptoms matter. Your medical history matters. And the other systems operating inside your body matter too.
For me, learning to stop looking at every symptom in isolation was one of the biggest changes in how I approached my health. The more we understand the conversations happening inside the body, the less chaotic they begin to feel.
